Pharmacy negligence and medication mistakes occur when pharmacists delegate their duties to employees who are not trained or legally allowed to perform these drug utilization reviews or provide adequate counseling. This often occurs in big chain pharmacies who fill millions of prescriptions and don't take the time to perform all their obligations to ensure their patrons are safe.
